A screening to identify your risk factors for heart disease – blood pressure, blood sugar, total cholesterol, HDL and LDL, triglycerides, weight and family history. Meet with a counselor to set goals for a healthy lifestyle. Participants receive a free initial screening and ONE free six-month follow-up screening. 12-hour fasting required before appointment. Appointment required.
